Anulus Tears in the Intervertebral Disc Degeneration

Abstract
Regarding the degenerative change of intervertebral disc and vertebral joint complex. The importance of discrete anular tear is widely accepted. But its initiation and prcess of propagation are still under study.
We investigated the animal model to verify the hypothesis that discrete anular tear would lead to secondary degenerative change on the intervertebral joint complex. And to observe the influence of the degeneration of disc complex on the facet
joint.
Among six one-year-old dogs. We analyzed twelve intervertebral discs of four dogs. Because two were lost due to the postoperative complication. We took simple radiography and MRI of the lumbar spine preoperatively. After the partial laminectomy.
The
anuli of L6-L7 and L7-S1 were incised transversely with 5mm in depth under the general anesthesia. Then. Plain X-ray and MRI were taken with three months interval during breeding. Six 4.5mm-thick parasagittal sections were harvested for
histopathologic
examination after sacrifice of animals at postoperative 6.9 and 12 months respectively.
The degenerative changes including narrowing of intervertebral disc space were noticed by the simple radiography, MRI and histopathologic examination from 9 months after operation. Especially. The MRI showed the early degenerated and protruded
type
of
herniation at 9 months and the severe from at 12 months after operation. Despite the outer anular healing process, the disc degeneration was initiated and propagated irreversibly.
Thus, we confirmed the hypothesis that the discrete tear might be one of the initiating factors to the processes of disc degeneration. However, the anular tears and partial laminectomy did not influence the facet joint degeneration in a year.
